Though generally understood as involving an array of factors, from genetic predispositions to environmental circumstances, it has turned out to be impossibly challenging to bring these factors together. The stakes of the challenge are considerable, since (in 2010) major depressive disorder (MDD) ranked as the second leading cause of disability worldwide and as the eleventh leading cause of global burden of disease (Ferrari et al. 2013).
